In the film "Shutter Island" i couldn't understand what happened at the very end..?

I understand that the Detective came out of the illusion and admitted
that he killed his wife, he was cured and at the very end he got back
in illusion again, but he said
"It's better to die as a good man rather live as a daemon" so was he
cured and was acting as he was in the loop again ?
